Summary
Lauren used nicotine patches for Long COVID, starting with half of a 7mg patch (3.5mg) worn intermittently for 7-10 days at a time with a few days off, then shifting in October 2025 to near-daily half-patch use for about two months, before cutting back in late December 2025 to just one-eighth of a 7mg patch daily after side effects appeared. Eighteen months into her Long COVID illness she had been experiencing severe POTS symptoms, an elevated heart rate with minimal exertion, and exhaustion so severe she couldn't keep her eyes open even after 12-plus hours of sleep. On the patches she gained more consistent energy, stopped needing afternoon naps, and could get through a full remote workday without her usual evening headaches, fatigue, and PEM. After about two months of continuous use she developed daily heart palpitations, nightly muscle twitches, and other side effects, and only after reducing her dose to an eighth of a patch did those adverse effects resolve, meaning the effective dose had to be scaled back to remain sustainable.
