Summary
Case 2, a 45-year-old woman about 11 months into Long COVID with orthostatic intolerance, tachycardia, short-term memory loss, word-finding trouble, fatigue, PEM and MCAS symptoms, took a 15-day extended course of Paxlovid (nirmatrelvir/ritonavir) in February 2023 on top of her existing low-dose naltrexone, cetirizine, norethindrone acetate-ethinyl estradiol-iron, lumbrokinase, serrapeptase and palmitoylethanolamide. During the course her energy and blood pressure stabilized around 145/80 and her orthostatic intolerance resolved, though she developed constipation and MCAS flares as side effects. Two weeks after finishing Paxlovid she reported improved energy, strength and mental clarity with complete resolution of orthostatic intolerance that had not recurred as of July 2023, and her MCAS was brought under control with added H1/H2 blockers, so the gains held rather than faded.
