Summary
Case 4, a 51-year-old man about 6 months into Long COVID with fatigue, daily headaches, brain fog, mental exhaustion, PEM, racing heart, palpitations, tinnitus and visual/sensory disturbances, took a 15-day extended Paxlovid course in March 2023 alongside concurrent valacyclovir for EBV reactivation and glutathione/B-complex injections. By the end of the first week his energy improved to his baseline for several days, with PEM, paresthesia, headaches and tinnitus all easing. The benefit faded about four weeks after stopping Paxlovid and two weeks after stopping valacyclovir, when crashes resumed; restarting valacyclovir brought improvement back within 24 hours, and after adding low-dose naltrexone in mid-June he reported being back to his pre-COVID baseline on all symptoms by mid-July 2023.
